The concept of patience describes a person's ability to make prolonged efforts towards future goals, and his or her ability to consider long-term future consequences. Clearly, patience is a capacity that comes by degrees. On the following pages, a person will be said to be patient to the extent that his actions are motivated by future consequences. Enormous gaps between HIV burden and health care availability in low-income countries raise severe ethical problems. This article analyzes four HIV-priority dilemmas with interest across contexts and health systems. We explore principled distributive conflicts and use the Atkinson index to make explicit trade-offs between health maximization and equality in health. We find that societies need a relatively low aversion to inequality to favor treatment for children, even with large weights assigned to extending the lives of adults: higher inequality aversion is (...) needed to share resources equally between high-cost and low-cost treatment; higher inequality aversion is needed to favor treatment rather than prevention, and the highest inequality aversion is needed to favor sharing treatment between urban and rural regions rather than urban provision of treatment. This type of ethical sensitivity analysis may clarify the ethics of health policy choice. (shrink)

Next SectionBackground International and national agencies play a major role in setting HIV care-and-treatment priorities in low-income-countries. Little is known about priority setting at lower health-system levels. The objective of this article is to explore experiences of HIV priority decisions, at what levels these decisions are made and how they might influence the distribution of health benefits in a high-endemic region in Tanzania. Methods This is a qualitative study using observations, key documents and semistructured focus-group and individual interviews (43) with (...) health workers, patients and administrators at one regional and one district hospital. The analysis was based on an editing analysis style. Results Health workers did not perceive themselves as bedside rationing agents and they reported following national recommendations and felt they have little impact on important priority decisions. Health benefit distributions were largely determined by priority decisions made at a national level. External factors, such as eligibility criteria, inadequate funding and barriers to access seemed to play a major role in the actual distribution of health services and benefits in the region. Patient groups who were actually given high priority were the sickest patients, those living near a facility and those who could afford long journeys and frequent visits. Low-priority groups were those experiencing other co-morbidities or treatment failure, children, asymptomatic patients and the poorest. Conclusions/significance The interaction of priority setting across health-system levels and their impact on distribution of health outcomes in the population has been underestimated. The distributional pattern of health outcomes calls for further normative assessment. (shrink)

Global health is at a crossroads. The 2030 Agenda for Sustainable Development has come with ambitious targets for health and health services worldwide. To reach these targets, many more billions of dollars need to be spent on health. However, development assistance for health has plateaued and domestic funding on health in most countries is growing at rates too low to close the financing gap. National and international decision-makers face tough choices about how scarce health care resources should be spent. Should (...) additional funds be spent on primary prevention of stroke, treating childhood cancer, or expanding treatment for HIV/AIDS? Should health coverage decisions take into account the effects of illness on productivity, household finances, and children's educational attainment, or just focus on health outcomes? In this article, we propose the Fair Priority Model for COVID-19 vaccine distribution, and emphasize three fundamental values we believe should be considered when distributing a COVID-19 vaccine among countries: Benefiting people and limiting harm, prioritizing the disadvantaged, and equal moral concern for all individuals. The Priority Model addresses these values by focusing on mitigating three types of harms caused by COVID-19: death and permanent organ damage, indirect health consequences, such as health care system strain and stress, as well as (...) economic destruction. It proposes proceeding in three phases: the first addresses premature death, the second long-term health issues and economic harms, and the third aims to contain viral transmission fully and restore pre-pandemic activity. -/- To those who may deem an ethical framework irrelevant because of the belief that many countries will pursue "vaccine nationalism," we argue such a framework still has broad relevance. Reasonable national partiality would permit countries to focus on vaccine distribution within their borders up until the rate of transmission is below 1, at which point there would not be sufficient vaccine-preventable harm to justify retaining a vaccine. When a government reaches the limit of national partiality, it should release vaccines for other countries. -/- We also argue against two other recent proposals. Distributing a vaccine proportional to a country's population mistakenly assumes that equality requires treating differently situated countries identically. Prioritizing countries according to the number of front-line health care workers, the proportion of the population over 65, and the number of people with comorbidities within each country may exacerbate disadvantage and end up giving the vaccine in large part to wealthy nations. The lipid‐bilayer component of cell membranes is an aqueous bimolecular aggregate characterized by a heterogeneous lateral organization of its molecular constituents. The heterogeneity may be sustained statically as well as dynamically. On the basis of recent experimental and theoretical progress in the study of the physical properties of lipid‐bilayer membranes, it is proposed that the dynamically heterogeneous membrane states are important for membrane functions such as transport of matter across the membrane and enzymatic activity. All parties involved in researching, developing, manufacturing, and distributing COVID-19 vaccines need guidance on their ethical obligations. We focus on pharmaceutical companies' obligations because their capacities to research, develop, manufacture, and distribute vaccines make them uniquely placed for stemming the pandemic. We argue that an ethical approach to COVID-19 vaccine production and distribution should satisfy four uncontroversial principles: optimising vaccine production, including development, testing, and manufacturing; fair distribution; sustainability; and accountability. All parties' obligations should be coordinated and mutually consistent. For (...) instance, companies should not be obligated to provide host countries with additional booster shots at the expense of fulfilling bilateral contracts with countries in which there are surges. Finally, any satisfactory approach should include mechanisms for assurance that all parties are honouring their obligations. This assurance enables countries, pharmaceutical companies, global organisations, and others to verify compliance with the chosen approach and protect ethically compliant stakeholders from being unfairly exploited by unethical behaviour of others. Those who endorse the view that death is in some cases bad for the deceased—a view that, as I shall explain, has considerable bearing on many bioethical issues—need to address the following, Epicurean question: When is death bad for the one who dies? The two most popular answers are "before death" (priorism) and "after death" (subsequentism). Part of the support for these two views consists in the idea that a third answer, "at no time" (atemporalism), makes death unsatisfyingly different from (...) other evils. I argue that this objection is mistaken, and that priorism and subsequentism face problems that atemporalism avoids. Moreover, I argue that if it is nonetheless insisted that we must find a time at which my death is bad for me, we can appeal to periods that begin before my death and end after my death. COVID-19 vaccines are likely to be scarce for years to come. Many countries, from India to the U.K., have demonstrated vaccine nationalism. What are the ethical limits to this vaccine nationalism? Neither extreme nationalism nor extreme cosmopolitanism is ethically justifiable. Instead, we propose the fair priority for residents framework, in which governments can retain COVID-19 vaccine doses for their residents only to the extent that they are needed to maintain a noncrisis level of mortality while they are implementing reasonable public (...) health interventions. Practically, a noncrisis level of mortality is that experienced during a bad influenza season, which society considers an acceptable background risk. Governments take action to limit mortality from influenza, but there is no emergency that includes severe lockdowns. This “flu-risk standard” is a nonarbitrary and generally accepted heuristic. Mortality above the flu-risk standard justifies greater governmental interventions, including retaining vaccines for a country's own citizens over global need. The precise level of vaccination needed to meet the flu-risk standard will depend upon empirical factors related to the pandemic. This links the ethical principles to the scientific data emerging from the emergency. Thus, the FPR framework recognizes that governments should prioritize procuring vaccines for their country when doing so is necessary to reduce mortality to noncrisis flu-like levels. But after that, a government is obligated to do its part to share vaccines to reduce risks of mortality for people in other countries. We consider and reject objections to the FPR framework based on a country: having developed a vaccine, raising taxes to pay for vaccine research and purchase, wanting to eliminate economic and social burdens, and being ineffective in combating COVID-19 through public health interventions. Cognition, Education, and Communication Technology presents some of the recent theoretical developments in the cognitive and educational sciences and implications for the use of information and communication technology (ICT) in the organization of school and university education. Internationally renowned researchers present theoretical perspectives with proposals for and evaluations of educational practices. High-spin states in the odd-odd N = Z nucleus Co-54 have been investigated by the fusion-evaporation reaction Si-28(S-32,1 alpha 1p1n)Co-54. Gamma-ray information gathered with the Ge detector array Gammasphere was correlated with evaporated particles detected in the charged particle detector system Microball and a 1 pi neutron detector array. A significantly extended excitation scheme of Co-54 is presented, which includes a candidate for the isospin T = 1, 6(+) state of the 1f(7/2)(-2) multiplet. The results are compared to large-scale shell-model (...) calculations in the fp shell. Effective interactions with and without isospin-breaking terms have been used to probe isospin symmetry and isospin mixing. A quest for deformed high-spin rotational cascades proved negative. I agree with Scott A Anderson1 and Rosalind J McDougall2 that many prostitutes suffer significant harms, and that these harms must be taken seriously. Having a background in public outreach for sex workers, I share this concern wholeheartedly.In the article to which Anderson and McDougall respond,3 I ask why prostitutes are harmed: are prostitutes harmed because prostitution itself is harmful or because of contingent ways in which prostitutes are socially and legally treated? This is an important question, since if the (...) latter is the case, then the widespread moral and legal campaign against prostitution, rather than being a legitimate response to something harmful, is itself the source of much suffering and distress. In my article, I argue at length that it is indeed our social and legal treatment of prostitutes that is the dominant source of harm.Neither Anderson nor McDougall seems to take issue with my rebuttal of the nine most central arguments in favour of the view that prostitution itself is harmful. They do, however, raise a number of more general issues, and I shall now address these.A worry raised by Anderson is that in defending the view that prostitution itself is not harmful, I am ‘abstracting away from key facts about the context in which prostitution takes place’, and as such, I ‘fail to recognize how contextual, historical and social’ prostitution really is. I think this is a good objection — so good, in fact, that I raise and discuss it in the article. I formulate the objection as follows: "[The objection] states that my argument is utopian: that prostitution is a complex practice deeply entrenched in a long line of other social and psychological issues, such as gender inequality, poverty, power hierarchies and exploitation, and that … ". Until the late 19th century scientists almost always assumed that the world could be described as a rule-based and hence deterministic system or as a set of such systems. The assumption is maintained in many 20th century theories although it has also been doubted because of the breakthrough of statistical theories in thermodynamics (Boltzmann and Gibbs) and other fields, unsolved questions in quantum mechanics as well as several theories forwarded within the social sciences. Until recently it has furthermore been assumed (...) that a rule-based and deterministic system was also predictable if only the rules were known, but this assumption has now been undermined by modern chaos-theory describing rule-based and deterministic, but unpredictable systems, while catastrophe-theory delivers a set of types describing various kinds of instability and conditions for the stability of a given system. Hence the main trait in the theoretical development in the 20th-century science can be described as a basic modification and limitation of some of the fundamental and strong assumptions forwarded in the previous epochs of modern science. Ironically, the very same process has been a process in which the human capacity to intervene in nature has expanded dramatically and mainly with the help of the very same theories, and not least because they allow nature to be described and made manipulable on a lower level and a more fine-grained scale. While the overall theoretical consistency between the various theories has gone, the reach of human intervention in nature has increased based on quite new dimensions whether in the area of physics (e.g.: energy technologies, chemical technologies, nanotechnologies etc.) or biology (ge¬netic manipulation) or in the area of psychology, sociology and culture (artificial simulations of mental processes, new means of communication, implying changes in the social infrastructure and cultural behaviour etc.). While some of these changes and new conditions can be reflected from within the conceptual framework of rule-based systems, albeit more complex than formerly recognized, others seem to give rise to the question of whether there are »systems« and relations between different systems in the world which are not rule-based? For instance, it seems to be obvious that the notion of instability represents a major conceptual break with former theories of rule-based systems, as the stability of the latter is an axiomatically given property implied in the very notion of rule-based systems, while instability can only be the result of external influence which should be explained as the result of another rule-based sy¬stem. While there are no difficulties implied concerning the stability of rule-based systems, the notion of unstable states of a system raises the question of how there can be a system at all if there are no invariant stabilising principles? This is the first question which I will address. And I shall do so by taking two examples of such systems as my point of departure. The first example will be the computer and the second will be ordinary language. In both cases I will argue that the stability of these systems (which are both defined by the existence/presence of human intentions) are provided by the help of - differently organised - redundancy functions which both allow the maintenance of systems in unstable macro-states, suspension of previous rules, underdetermination and overdetermination and generation/emergence/creation of new rules more or less independent of previous rules by the help of optional recursions to the permanently accessible underlying levels as for instance the level of binary representation in computers. Since the notion of redundancy is both controversial as such and often avoided, the concept is discussed (as defined in Claude Shannon's mathematical theory of information and in the semiotic framework of J. J. Greimas) and leading to a more general definition in which the redundancy functions serve to overcome noisy conditions, but at the cost of rule-based stability, determination, and predictability. A second question will be how the notion of rule-generating systems relates to the notion of anticipatory systems and it will be argued that rule-generating systems share some features with an¬ticipatory systems and that the former from a certain viewpoint can be seen as a subclass of the latter, although anticipative features are not necessarily a part of the definition of rule-generating systems. On the other hand, it will be discussed whether anticipatory systems which are not rule-generating systems can exist and it will be argued that the capacity to anticipate is strongly limited if it is not part of a rule-generating system. Therefore, it is concluded that the most powerful anticipatory systems need to be rule-generating systems. (shrink)

The paper presents and discusses phenomenological facts about perceptual spaces and percepts, but ends with a few thoughts about possible causal explanations of such spaces. The overarching double-sided hypothesis claims that - from a phenomenological point of view - each individual animal has at each consciously perceived moment of time a sense-modality-neutral perceptual space, and that these perceptual spaces are so-called container spaces. This means, to be concrete, that blind persons, deaf persons, and all perceptually non-handicapped persons have the same (...) kind of phenomenological perceptual space, a sense-modality-neutral container space. Also, I claim that there might be thin external relations. We study unification problem and problem of admissibility for inference rules in minimal Johanssonsʼ logic J and positive intuitionistic logic IPC+. This paper proves that the problem of admissibility for inference rules with coefficients is decidable for the paraconsistent minimal Johanssonsʼ logic J and the positive intuitionistic logic IPC+. Using obtained technique we show also that the unification problem for these logics is also decidable: we offer algorithms which compute complete sets of unifiers for any unifiable formula. The present paper is to be considered as a sequel to [1], [2]. It is known that Johansson's minimal logic is not uniform, i.e. there is no single matrix which determines this logic. Moreover, the logic C J is 2-uniform. It means that there are two uniform logics C 1, C 2 (each of them is determined by a single matrix) such that the infimum of C 1 and C 2 is C J. Logic isn’t special. Its theories are continuous with science; its method continuous with scientific method. Logic isn’t a priori, nor are its truths analytic truths. Logical theories are revisable, and if they are revised, they are revised on the same grounds as scientific theories. These are the tenets of anti-exceptionalism about logic. The position is most famously defended by Quine, but has more recent advocates in Maddy, Priest, Russell, and Williamson. Although these authors agree on many methodological issues about logic, (...) they disagree about which logic anti-exceptionalism supports. Williamson uses an anti-exceptionalist argument to defend classical logic, while Priest claims that his anti-exceptionalism supports nonclassical logic. This paper argues that the disagreement is due to a difference in how the parties understand logical theories. Once we reject Williamson’s deflationary account of logical theories, the argument for classical logic is undercut. Instead an alternative account of logical theories is offered, on which logical pluralism is a plausible supplement to anti-exceptionalism. (shrink)

The paper presents a revised view of quantum mechanics centered on the notion (“genuine fortuitousness”) that the click in a counter is a totally lawless event, which comes by itself. A crucial point is the distinction between events on the spacetime scene and the content of the symbolic algorism. A revised conception of matrix variables emerges, by which such a variable, as part of a whole, does not have a value, under any circumstance. This conception is at variance with that (...) of indeterminate variables. A matrix variable not having a value does not enter spacetime and is not a measurable quantity, but manifests itself by a click in a counter with the remarkable property of having an onset, a beginning, from which the click develops. The individual click with its immense complexity is unique and lawless, even beyond probability. The notion of probability only applies to clicks in low resolution, and the completeness of a probabilistic theory is thereby seen in a new perspective. The genuinely fortuitous click is not produced by the impact of a particle, nor caused by an event in the source prior to the click in the counter. Indeed, there are no particles on the spacetime scene. The theory is thereby liberated from notions, which go with particles having indeterminate variables, and which have given to quantum mechanics the image of an unfathomable theory. With no particle as intermediary, the connection between source and detector is non-local, as is the entire theory, which deals exclusively with distributions of clicks. The locality permeating quantum mechanics is a symbolic one. The wave function enters in the sole role of encoding the probability distributions of clicks. The quest to understand the occurrence of the click in terms of the evolution of the wave function loses its meaning. However, click distributions in low resolution can be analyzed in terms of the connection between click distributions for different sets of counters, as given by the wave function. With increasing resolution, the probabilities, and the wave function, gradually lose significance, whereby the onset remains beyond reach. Thus, the downward path from events on the spacetime scene does not extend beyond the onset. The notion that quantum mechanics deals with particles (or fields) is rooted in the historical evolution but appears unable to accomodate genuine fortuitousness. The latter concept is given its due place by fully accepting the abstract nature of the matrix variables. (shrink)

'Business Ethics and Organizational Values' operates on three levels - society, organization and values. First, the dynamics of modern society are analyzed. Secondly, organizations are viewed as autopoietic systems constantly engaged in value debates with their stakeholders. Thirdly, values are placed in a business context.

Anti-exceptionalism about logic is the Quinean view that logical theories have no special epistemological status, in particular, they are not self-evident or justified a priori. Instead, logical theories are continuous with scientific theories, and knowledge about logic is as hard-earned as knowledge of physics, economics, and chemistry. Once we reject apriorism about logic, however, we need an alternative account of how logical theories are justified and revised. A number of authors have recently argued that logical theories are justified by abductive (...) argument (e.g. Gillian Russell, Graham Priest, Timothy Williamson). This paper explores one crucial question about the abductive strategy: what counts as evidence for a logical theory? I develop three accounts of evidential confirmation that an anti-exceptionalist can accept: (1) intuitions about validity, (2) the Quine-Williamson account, and (3) indispensability arguments. I argue, against the received view, that none of the evidential sources support classical logic. (shrink)

This paper asks how anti-nuclear activists form collectives that are able to act collectively. It argues that shared interests and collective identities only insufficiently explain the emergence of collective action. Alternatively, the paper investigates meeting talk of German anti-nuclear groups where activists discuss proposals for collective action. Based on audio recordings, a sequential analysis of activists’ deliberations traces the transformation of vague ideas into concrete and collectively agreed to proposals. It is shown how the process by which activists reach a (...) common understanding about a particular protest activity—as something that is being talked about here and now—and the process of becoming an acting collectivity—a group that is ready to carry out this protest activity in the future—are interdependent. Activists make use of “indexicality” to introduce specifically vague proposals, enabling others to respond and contribute to the emerging proposal while group agency is suspended. At first, the authorship of proposals is minimized. Step by step, vague proposals are specified by meeting participants until a “change in footing” marks group consensus. While the group emerges as the author and principal of proposals through this process, the paper shows that implementation relies on individual principals again who take responsibility during and after the meeting. We argue that deliberative decision-making that is inclusive, transparent and accountable can contribute to more trustworthy and legitimate decisions on difficult ethical questions and political trade-offs during the pandemic and beyond.
